Description:
The Interdisciplinary Practice Model coordinator is the Sutter Coast Hospital (SCH) champion of clinical transformation as it relates to the new care delivery model. He/she is the leader, mentor and role model to facilitate the development of a professional interdisciplinary practice environment through adoption of the Interdisciplinary Practice Model and electronic health record (EHR) clinical documentation at each campus. The SCH Interdisciplinary Practice Model Coordinator will collaborate with the Sutter Health Interdisciplinary Practice Advancement Council (IPAC) System Leader, Patient Care executive, EHR physician and nurse champions, Clinical Department Directors/Managers, Physicians and Nursing staff leaders to identify and to implement the Clinical Practice Model Resource Center (CPMRC) infrastructure and tools that support the scope and processes of professional interdisciplinary practice and quality care. The incumbent works closely with educators and nursing leadership to coordinate and assure appropriate training for staff. The Interdisciplinary Practice Model Coordinator role models the principles of inter-disciplinary practice through working relationships with individuals and on teams.

Serving the greater Stanislaus County with 423 plus acute care beds, Memorial Medical Center provides a full continuum of care including 24-hour emergency care, inpatient and outpatient surgery, obstetrics, pediatrics, nuclear medicine, dialysis therapy, diagnostic imaging, intensive care, physical therapy, and home health. Specialized care includes Memorial Regional Cancer Center and Memorial Heart Institute.
Minimum Requirements:
* Current and active California registered nurse license.
* Bachelor's degree in nursing or related clinical field.
* Specific experience or training in change management processes, clinical quality programs, advanced clinical education preferred.
* Thorough knowledge of health care delivery; nursing practice and documentation requirements.
* Five (5) years hospital based, clinical health care experience.
* Commitment to interdisciplinary practice and problem solving is required.
* Proficiency in Microsoft Work, Excel, Access, and Outlook.
* Conceptual, analytical, negotiation, and problem solving skills.
* Demonstrates excellent critical thinking skills.
* Exceptional communications skills, both oral and written.
* Dynamic leadership skills and excellent facilitation skills.

* Must be able to travel in accordance with any requirements of your job, (must possess valid California Driver's License and upon request proof of liability insurance).
Preferred Qualifications:
* Experience with the CPMRC practice framework is highly desired.
* Demonstrated ability in leading organization wide projects is highly desired.
* Master's level training in health care administration, nursing or business is preferred.
* Experience with Electronic Health Record systems is highly desired.
